Responsible for performing day to day implementation and follow up case management for patients in Cardiac Rehab. Provides individualized rehab program to meet personal and family needs of patients. Position is responsible for outpatient classes. Position requires understanding of cardiovascular disease. MINIMUM EDUCATION: Bachelor�s Degree in Exercise Physiology or related field from an accredited college or university MINIMUM EXPERIENCE: 12-week internship in cardiac rehabilitation PREFERRED EXPERIENCE: 1-2 years' experience with exercise prescription in a clinical setting PREFERRED CERTIFICATIONS/LICENSURE: American College of Sports Medicine � Clinical Exercise Physiologist
