Summary of Position
• Empower participants to develop the skills and confidence required to effectively self-manage their health and wellness while implementing WellSpark’s Bio-Psycho-Social model of care.
• Perform outreach telephonically providing education/support, assisting with personalized goal setting, and encouraging the development of action plans; holistically supporting the mind-body connection.
• Foster high quality relationships while reducing healthcare cost utilization to maximize healthy lifestyle choices.
Principal Accountabilities
• Perform telephonic outreach to engage identified population to promote wellness by coaching, motivating, and educating to attain maximum mental/physical well-being.
• Facilitate promotion of wellbeing to aid in health literacy, reducing lifestyle risks and supporting readiness to change as they relate to health programs.
• Empower participants to become active in their own health by utilizing motivational interviewing, mind/body interventions and behavior change techniques to make ongoing healthy lifestyle choices.
• Provide education on proactive actions to support preventative care.
• Manage high-profile accounts for WellSpark.
• Connect participants with available community resources addressing social determinants of health and other referral sources. Serve as an advocate for resources while providing guidance in the selection of available choices.
• Make appropriate decision as to when a Nurse transfer or consult is appropriate.
• Actively participate in committees, projects, grand rounds, vendor relationships and other meetings that may arise.
• Work collaboratively with manager/supervisor for caseload panel management, call productivity audits, program
engagement, continual staff development and maintenance of team goals.
• Act as a mentor and support to fellow colleagues to maintain a highly functioning team.
• Maintain current “state of the art”, cutting edge working knowledge of his/her specialty, and related computer programs through continuing education, community resources and publications.
• Enter and maintain critical data into documentation system to ensure accurate and timely reporting.
• Manage a flexible work schedule to meet the needs of participants and optimize telephonic outreach rates.
• May be responsible for managing phone queues for service excellence.
• Perform other duties as directed or required.
Education, Training, Licenses, Certifications
• Bachelor’s Degree in health-related field (e.g., Nutrition, Exercise Physiology, Public Health)
• Certification in a Health Coaching/Health & Wellness Coach Certification through Wellcoaches preferred.
Relevant Work Experience, Knowledge, Skills, and Abilities
• 3 – 5+ years of relevant, professional work experience, including coaching experience.
• Experience with Cognitive Behavioral Coaching.
• Experience with managing a large volume of populations with various acuity levels.
• Additional experience/specialize training may be considered in lieu of degree requirements.
• Ability & willingness to travel to multiple work sites, and work some evenings, weekends, holidays, and off-hours to accommodate population needs.
• Experience in wellness related areas (weight management, nutrition, physical activity, health lifestyle behavior change, etc.).
• Proficient with computers (MS Office apps), electronic medical records systems, automated environment to support multiple technology platforms, and standard office equipment.
• Proficiency with the use of remote technology (Smartphone, wireless laptop, virtual conference tools, etc.).
• Strong communication skills (verbal, written, presentation, interpersonal) with all types/levels of audience.
• Strong time management and organizational skills.
• Ability to work effectively as an individual contributor or in a team environment.
• Experience in facilitating educational sessions, in person one-on-one and group coaching and via technology modalities.
• Ability to deliver content in an engaging way and adapt to audience needs and/or questions.
• Bilingual language skills.
• Possibility of cross-training between accounts.
• Reliable transportation for any on-site position.
